I think your riddle should read “Who is my mother’s sister’s only child’s daughter’s uncle’s aunt?”
Which would make the answer your mother’s sister.
I don’t know how to do arrows or anything connecting up but

My mother’s sister’s only child is my cousin because my mother’s sister is my aunt and my aunt’s only child is my cousin. My cousin’s daughter is my second-cousin. My second-cousin’s uncle is her father’s brother because her mother (my aunt) is an only child. My second-cousin’s father is not related to me, so neither is my second-cousin’s father’s brother. My second-cousin’s father’s brother’s aunt (my second-cousin’s uncle’s aunt) is my second-cousin’s great-aunt, and she is not related to me.
And the speaker can’t also be the uncle mentioned in the riddle, can s/he (especially if the speaker is female)?

If the speaker were male, the question as I ask it (without the grandmother bit), boils down to who is the uncle’s aunt. The uncle’s aunt would be his mother’s sister. “Who is my mother’s sister’s only child’s daughter’s uncle’s aunt?”

Right, but the question is “Who is my mother’s sister’s etc. ?” So the speaker can’t be the uncle, because the question says “my” and the uncle is my (aka the speaker’s) mother’s sister’s only child’s daughter’s uncle. And the uncle in the riddle isn’t even technically related to the speaker.
