I just had If I Needed Someone on 17% and I found that ever so slightly too loud... How can people cope with >40%? Granted IINS isn't the loudest song in the world...

Something to consider is that there isn't a universal volume for audio files ripped from CDs. I have noticed that some tools produce louder MP3's than others... so there are really no hard and fast rules other than take care and be aware of potential damage.

Those of us who like to listen to our music on the subway will note that the volume has to be up pretty damned high to block out all the ambient noise.

I used to listen to my music through earbuds a lot more than I do now. I hardly ever listen with ear buds unless I'm in public somewhere.

Why don't you try for noise isolation earphones or either try some fine quality in budget headphones(wireless with bluetooth headphones for convenience). Noise isolation one are really good for ears. If you are facing some serious hearing damage or issue related to this, than you must have it.
